Cheshire and Wirral Partnership NHS Foundation Trust provide health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of GP surgeries. 

 

My Health and Wellbeing Passport

The aim of this passport is to provide our staff with all the necessary information about the patient when they use our hospital services. The information that is supplied will help us to work in partnership with meeting the patient’s needs.

Some of the questions in the passport include:

• How the patient prefers to communicate

• Allergies

• Sensory needs

• Things the patient likes and dislikes

 

If this sounds like something you would benefit from using, please send an email with your name and phone number, and we will contact our Learning Disability, Dementia and Complex Care Specialist to go through the passport with you.

 

Who is this aimed at?

This is open to all patients who feel they would benefit from a Health and Wellbeing passport

 

When

This resource can be accessed 24/7 online

You can use your passport anytime you go to an NHS Foundation Trust hospital

 

Where

You can use this passport at any NHS Foundation Trust hospital

 

Cost

This service is free
