Family Nurse Partnership Wirral and Cheshire East

Family Nurse Partnership Wirral and Cheshire East is a home visiting programme for first-time young mums and families.

 

Before signing up to the programme one of our family nurses will schedule a home visit with mum-to-be (this visit can also be arranged in another venue if requested) to explain what the Family Nurse Partnership offers and what support looks like. If mum-to-be would like to take up the support, and if space is available she can do so. Our service is offered to all first time pregnant mums aged 18 and under. 

We also support:

  • First time pregnant mums aged 22 and under with specific vulnerabilities and needs. (Wirral)

  • First time pregnant mums aged 24 and under with specific vulnerabilities and needs. (Cheshire East)

What we do:

  • Help young mums-to-be have a healthy pregnancy.

  • Promote their child’s health and development.

  • Support positive parenting and attachment.

  • Support the parent’s own future plans and aspirations.

Intensive support is provided for first-time young mums, their partners (if agreed to by the young mum), and their babies up until their first child reaches 2 years of age. Each family has a dedicated family nurse for the length of the programme, which enables a strong relationship to develop during the early parenting journey. Our nurses are specifically trained and visits are regular either at home or in a venue that suits the family. The visit schedule changes during the programme in agreement with mum and can range from weekly to monthly visits.

Our family nurse replaces the health visitor whilst mum and her son/daughter are signed up to the FNP programme.

How to access the service

Anyone can refer in to the Family Nurse Partnership programme with the young mum’s permission. Young women can also self-refer.

All referrals need to include the following information:

  • Name, address and date of birth of the mum-to-be

  • Estimated due date of baby

  • School attended (if known)

*Please also include information regarding any other specific needs or vulnerabilities e.g. learning, social or communication difficulties, mental health issues, safeguarding issues or status, homelessness, lack of family support.
